You can Download the directory form  from the link provided hereunder.
1) Click the Link
2) Form will open in the browser
3) On Google Doc page there is a print button . Click the button or press Ctrl+P together to print the form
4) Fill the form in GUJARATI 
5) All the members including ladies (if they have their own Cell Phone) should be included in the form
7) Important : Members established outside India are also to be covered in the Telephone Directory. You can take xerox of the form received in the Samaj Utkarsh and fill that form separately with the information of your relative staying abroad.
6) Submit the form to any one of 7 centers or scan the form and email it to mjvtd5@gmail.com.
